This checkup includes:

• Physical examination and interview with a doctor
• Height and weight measurement
• Blood pressure measurement
• Visual acuity and colour vision test
• Urine test for albumin, sugar and blood
• Random blood sugar test (for 25 years old and above)
• Chest X-ray
• Completion of medical examination form

TERMS AND CONDITIONS

• Price is in Singapore dollars and inclusive of GST.
• Upon purchase, we will send to your email a redemption letter, that you can use to redeem your package. This letter can only be used for the package indicated.
• This letter must be presented upon registration at the clinic, together with your NRIC / Identification documents.
• Applicable at Health Check at Raffles Hospital, Level 2 only.
• Prior appointment is not required. You can walk in to the clinic from 8.30am to 4pm, Monday to Friday; 8.30am to 11am on Saturday.
• Items in the package are non-exchangeable and non-refundable, including unutilised portion.
• Additional test items required will be charged separately.
• You may transfer the package to another person. Please fill in the authorisation form accordingly.
• Not valid with other corporate programmes.
• Listed terms are subject to change without prior notice.
